One of my guys came in this morning to fill in an accident form

Description of incident
I had been to use the toilet in the services and on leaving felt intense pain in my nether regions. I rushed back to the toilet and discovered that I had been stung on the penis by a wasp

I have to fill in a form for elf un safe-tee giving the results of my investigation and the actions to be taken to prevent a further occurence.
