An issue but minor.  Many of the dead died because they ran to the nearest exit - at the back of the plane.

Main cause of death was complete brain failure by the pilot's gross failure to properly respond to an in flight emergency, other reports say the fire engines were inadequate, no foam and were not dispatched until after the accident

Someone's getting sacked in the morning but they should make him clean the pilot's trouser first - with his tongue

'An aircraft had to make an emergency landing after one engine caught fire and the other stalled due to a fuel additive error, a report has found.

The plane took off from Gatwick Airport on 26 February after an engineer using an internet calculator added 37 times too much of the chemical.'

The actual problem stemmed from maintenance work in Larnaca ( lol:) when the engineer there got his maths woefully wrong. 30kg per wing of a biocide called Kathon (with 6,200kg of fuel on top) whereas it should have been about 0.9kg per wing. Eventually clogged the combustion chamber swirl cups, after a few sectors.
