I've had the damn thing in my chest for 10 years now so no need to lie down just at the moment.
Yes it has "gone off" in that time and yes it hurts like hell when it does but it is better than the alternative (I think).
And to answer the next question yes it has to have a new battery every few years and yes they have to open my chest (local anaesthetic), take out the machine and replace it as it is a sealed unit. It is about the size of a pack of twenty cigs and weighs about 4 ozs. Much bigger than a pacemaker which is about the size of the old half crown.
