However why is this so bloody complicated......you are either male or female. The whole minefield about how you address a woman is so over the top now. Simplify the whole issue. It should not matter whether a woman is married or single, use the one term ( like you would do for Mr so and so ) . This unfortunately only leaves the option of Ms - and take away the inferences from this that by choosing this option you are some ardent feminist or lesbian
Likewise when filling in forms why the feck does it matter if you are single, married, divorced, separated etc - what the feck is that information used for if you are applying for something in the singular
